Preregistration Information Registration at the Center Nursing Services Prescriptions Insurance and Payment Information We have two main offices in Fort Wayne, Indiana, as well as many satellite locations. Please be prepared to give the following information to the scheduler whenever you call for an appointment:
If you have been treated by one of our physicians in the past, please attempt to continue treatment with him or her for new problems.
If you are seeing one of our physicians for the first time, you may have received a packet of forms to be used to complete your registration before your visit. It is important that you complete these forms bring them with you to your office visit. To cancel or change appointments call the scheduling desk and give your name, your doctor's name and the date and time of your appointment. Please give us 24 hours notice of cancellation if at all possible. There is a $75 charge for NEW patients who do not keep their scheduled appointment. Emergency patients will be worked into the late morning or late afternoon schedule. You may need to come to the Center and wait to be worked into the schedule.
If this is your first appointment, please plan to arrive on time to complete the registration process. Upon arrival please check in with the receptionist. Give her the patient's name, the physician's name, and the time of your appointment. You will be called to the registration desk to verify your preregistration information or have your record updated. Please be prepared to give the following information:
After you have seen the doctor please STOP at the scheduling desk to schedule any tests or return appointments. From there you will be directed to the cashier's desk. Payment is expected at the time of services. Cash, personal checks, Visa, Mastercard, Discover and American Express are accepted. You will be given a copy of an itemized superbill that you should attach to your own insurance form to file with your insurance company. If you are a member of an HMO Plan, please remember that you must have a referral form from your primary care physician each time that you are seen. If you do not have this authorization form, you will be expected to pay for the services rendered at the time of your visit. If your insurance plan calls for a copayment for the services being provided, the copayment is expected at the time of service.
Your physician's nurse will give you her name and telephone number at the time of your office visit. She will be your contact person for questions regarding your care and treatment and for prescription refills. Your physician's nurse will be happy to answer your questions or forward them to your physician if needed. She or your physician will get back to you as quickly as they can. If your nurse is not at her desk when you call, please leave a message for her on her phone recorder by giving the following information:
Routine prescriptions will be refilled only during normal office hours. Please make every effort to monitor your medications so that you do not run out of medicine after office hours or on weekends. Your physician may not always be available after office hours are over. The physician on call may feel that he can not order your medication for you if he is not familiar with your case. Prescription refills may be obtained simply by asking your pharmacist to contact our office or you may call the center between 8:30 a.m. and 4:30 p.m., Monday through Friday and speak with your physician's nurse. Be prepared to give:
Certain medications require written prescriptions and cannot be ordered by telephone. For after hours emergency refills, have your pharmacist contact the physician on call through the physician answering service at (260) 482-3654.
INSURANCE AND PAYMENT INFORMATION It is your responsibility to be familiar with the requirements and restrictions of your insurance coverage. Please review your insurance policy before your scheduled visit. Some insurance companies require prior authorization or pre-certification for many medical services. These services could be ordered by your physician in the course of your treatment. This is particularly true if you belong to a prepaid health plan such as an HMO or a PPO. Your coverage May also require that you return to your primary care physician for authorization for or completion of many lab tests or x-rays that are ordered. Some plans identify specific hospitals or medical care providers which are to be used if you expect full coverage under your insurance plan. Please provide this information to the scheduler if any hospitalization or tests are ordered. The physician and his staff will make every effort to assist you in fulfilling the requirements of your insurance company but the ultimate responsibility is yours.
